First Visit
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, surgery can be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
Please assist us by providing the following information prior to your consultation:

Your surgical referral slip and any x-rays, if applicable.

A list of medications you are currently taking.

Your dental and/or medical insurance. Our preferred policy is to process all paperwork prior to the patient’s arrival; online registration can facilitate this. We also ask that you provide insurance information prior to your appointment so that it can be verified.
You may preregister with our office by filling out our registration forms. When you are ready to begin, CLICK HERE to print or electronically fill out the online form. After you have completed the form, save the file and send it to drsilvamonterey@gmail.com or bring the completed paperwork with you to your appointment.
If you choose to email us your completed forms, we will have it available for your signature upon your arrival for your first appointment. The security and privacy of your personal data is one of our primary concerns and we take every precaution to protect it.
X-Rays
If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
IMPORTANT:
All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.
A preoperative consultation and examination is mandatory for patients undergoing IV anesthesia for surgery.
